Thank you for taking the CAT assessment. Your honest and heartfelt answers to the questions in this survey are invaluable for helping our congregation identify our next steps as an organization. This survey is anonymous, no one will know how you choose to answer the questions. Please carve out between 30 to 60 minutes for the completion of the survey. The survey must be completed in one sitting when doing the online version. If you would like more time to complete the survey, we can provide you with a paper copy. The term “Spiritual/Spirituality” is used often during this assessment. As there are many definitions of Spirituality, the CAT team recommends that you understand this question in terms of your connection with God, personal faith, deeply held beliefs, and actions directed by your faith. We encourage you to answer based on what your understanding of your spirituality is at this present time, not what you think might be the right answers for anyone else.
